Overview:
We’re seeking a Nurse Practitioner to provide comprehensive outpatient primary care to adult patients, with a strong emphasis on geriatric populations and chronic disease management. This role focuses on longitudinal care for conditions such as diabetes, hypertension, cardiovascular disease, and obesity, while incorporating remote patient monitoring and chronic care management into daily practice. This role operates within a physician-led, team-based environment with a strong focus on collaboration, data-driven care, and clinical excellence.

Responsibilities:
- Provide primary care to adult patients (18+), with a heavy focus on geriatric populations and chronic disease management
- Manage approximately 3 patients per hour (~21 patients per day) while delivering high-quality, efficient care and maintaining accurate EHR documentation
- Utilize remote patient monitoring (RPM) and chronic care management (CCM) programs, including reviewing and analyzing patient data, conducting follow-ups, managing lab results, and performing patient outreach
- Deliver longitudinal care by developing and managing ongoing treatment plans for chronic conditions
- Collaborate closely with physicians, medical assistants, and clinical support staff across clinic locations in a team-based care model

Requirements:
- Active, unrestricted Nurse Practitioner license in Indiana (or ability to obtain) and MSN from an accredited program
- Minimum 3 years of experience in primary care, internal medicine, or family practice preferred
- New graduate NPs with strong RN experience (2+ years) in ICU, cardiovascular, telemetry, or other high-acuity settings will be considered
- Experience managing chronic conditions in adult or geriatric populations
- Comfort working in a fast-paced outpatient setting with a productivity expectation of 3 patients per hour
- Ability to work across multiple clinic locations and use a personal vehicle for transportation
- DEA license and prescriptive authority (or ability to obtain)
