Overview:
This role is part of a physician-led, value-based primary care model focused on older adult patients under Medicare Advantage. Physicians work with a capped patient panel and a dedicated interdisciplinary care team, emphasizing prevention, care coordination, and long-term health outcomes over volume. The position offers a predictable schedule and a strong focus on physician wellness.
Responsibilities:
- Deliver comprehensive primary care to older adult patients, serving as both a clinician and a health coach guiding lifestyle and long-term health improvements
- Coordinate specialty and hospital care while maintaining active involvement in patient management and care gap closure
- Lead and collaborate with an interdisciplinary team including Medical Assistants, Care Coordinators, and Care Facilitators
- Supervise Advanced Practice Providers (APPs) and partner with medical and operational leadership to achieve quality and cost objectives
Requirements:
- MD or DO with Board Eligibility or Board Certification in Internal Medicine, Family Medicine, or Geriatrics
- Active or eligible state medical license and active DEA license
- Commitment to value-based, team-oriented, patient-centered care
- 1–2 years of experience in a primary care setting preferred
